	ApHC UK Application Form for a Veterinary Certificate for a Colt/Stallion A document to be completed by an independent veterinary surgeon, and required before acceptance of a colt/stallion onto the ApHC UK register and which needs to be completed and sent to ApHC UK prior to inspection
	ApHC UK Overstamp Registration Application Form A single sided A4 document used in conjunction with the registration for ApHC UK to overstamp an existing passport supplied by another issuing office.
	ApHC UK Registration Application Form A two part document for the Application for a passport & registration into ApHC UK. One registration form and one silhouette form to be completed.
	ApHC UK Stallion Breeding Report Form A stallion owners cover note log of all mares covered and their details. A single sided A4 document.
	ApHC UK Stallion Licence Form A 5 star vetting form for registered colts/stallions. This form is necessary if you wish for the stallions progeny to be eligible for ApHC UK registration. A three part document that includes a silhouette, a veterinary surgeon's declaration and an owner's declaration.
	ApHC UK Stallion Outcross Breeding Report Form A stallion owners (of outcross stallions only) cover note log of all mares covered and their details. A single sided A4 document.
	ApHC UK Statutory Declaration as to Loss of Documents A single sided A4 document to be used if registration documents have been lost in the post or by other means and also required when a duplicate passport is requested due to loss, misplacement etc. Requires a solicitors signature.
	ApHC UK Statutory Declaration to Current Ownership A declaration form to be used when transferring ownership when no documentation has been supplied by the seller.
	ApHC UK Transfer Form To be used following a sale or similar transaction for change of ownership. Both seller and buyer signatories required.
	Breeds Pack 1 Breeders pack containing multiple forms for Stallion Licences, Registration Forms and a Registration Rule booklet.
	Breeds Pack 2 Breeders pack containing mulitiple forms for registration and a registration rule booklet.
